During eye consultations, most patients
want their eyes to be bigger and more refreshing than they are now, so when I
inquire about the desired eyelid line, they usually say outline. Still,
recently, they preferred semi-outline because the outline might look
overwhelming. But at the same time that you want semi-outline or outline
eyelids, you're also thinking, "Won't I end up with sausage eyelids if I
do this?" So, they ask for my advice in their consultation because they
are worried that it won't suit them and will be overwhelming.
Do you necessarily have to go with a
semi-outline or outline for big, refreshing eyes?
The answer is no. Patients bring photos of
their favorite celebrities to the consultation, such as Iz One Kim Minjoo, Espa
Karina, and Red Velvet Irene, among the actresses Shin Se-Kyung and Kim
Yoo-Jung, and Kang Tae-Ri and Im Bora whom I have seen a lot in recent
consultations. When you look at their eyes, they all have big, refreshing eyes,
and the most important thing is that their double eyelid lines are all
in-outline.
Of course, there are also celebrities with
large, refreshing eyes who have semi-outline or outline eyes, such as
BLACKPINK's Jenny and IVE's Jang Won Young.
Whether you have in-outline, semi-outline,
or outline eyes, what makes them look big and cool is determined by other parts
of the eye than the eyelid line, so let's look at what parts of the eye make
them look big and clear.
Common characteristics of celebrities who bring in photos for consultation
1. Their eyes look clear and bright
The clear appearance of the eyes is related
to the exposure of the pupil. Generally, 80-90% or more pupil exposure is
considered beautiful. If you look at the picture of celebrities with big, clear
eyes, you can see that the pupil is touching the upper eyelid or the upper
eyelid is only slightly covering the top of the black eye.
If you have good oculi force, your pupils
will be more exposed, and these eyes will look wide open. Bright eyes give the
impression of brightness and clarity, which is generally perceived as a
positive image.

eyelid revision, she also had a blepharoplasty and upper canthoplasty, and you

2. Refreshing Eyes Left and Right
Eyes tend to look more open when they are
divided into five sections, usually in a ratio of 1:1:1:1:1:1 horizontally.
Even if the proportions aren't exactly
right, lengthening the horizontal length of your eyes can make them feel more
extensive and more refreshing. To increase the flat size of your eyes, you need
a canthoplasty, which is the epicanthoplasty, upper canthoplasty, and dual
canthoplasty (back and lower canthoplasty).
Of these, dual canthoplasty can help soften the impression by lowering the raised lateral canthus and enlarging the size of the outer white of pupil. If the lateral canthus is too high, the impression may appear strong, and the eye size may seem small and stuffy.
This patient had natural adhesions and
underwent non-incisional ptosis correction, epicanthoplasty, and dual
canthoplasty.
You can notice that the non-incisional
ptosis has increased the exposure of the pupil, making the eyes appear clearer,
and the horizontal length of the eyes has been lengthened due to the
epicanthoplasty and dual canthoplasty.
The eye angle created by the inner and
outer canthus is called the 'Mongolian slope,' the higher the Mongolian slope,
the fiercer the eyes look. After the surgery, the corners of the eyes were
lowered by using a dual canthoplasty, reducing the montage's angle and
increasing the eyes' horizontal width.

Dual Canthoplasty is recommended to be
performed together rather than separately, as the upper and lower eyelids are
attached to the corner of the eye, so it is necessary to release the fusion of
the upper and lower eyelids through the canthoplasty while lowering the elastic
lower eyelid through the canthoplasty to widen the angle of the corner of the
eye and keep the canthoplasty without adhesion.
In addition, if the dual canthoplasty is
performed alone, it may still look stuffy after surgery because the canthus
angle is narrow, and the area stretched is not wide. Still, if the dual
canthoplasty expands the back and bottom simultaneously, the canthus will look
refreshing and elongated.
The
Right Line for You Is Important
Patients think semi-outline or outline eyes
are more prominent and refreshing, perhaps because they often mistake the
eyelid line's height for the eye's size.
The patient in the picture above has
changed the line from semi-outline to in-outline. Along with the incisional
double eyelid surgery and ptosis correction, you can see more pupils than
before, having the eyes look much more significant, refreshing, and clear.
This patient also changed the line from
outline to in-outline and had an incisional revision and ptosis correction. The
sausage eyelids and rather sleepy eyes before surgery transformed into clear
and refreshing eyes after surgery.
Due to the structure of the Asian eye, when
trying to create an outline or semi-outline, it is sometimes necessary to
excessively tear the Mongolian fold or have the line high. It would be best to
be careful with a high line, as it can create a sausage-eye look or look awkward.
However, creating an in-outline with a
clear line to the front of the eye will give the illusion of the eye's size up
to the level of the eyelid, so you can create the illusion of larger eyes
without the need for surgery with a semi-outline or outline, and create a
natural, unobtrusive look.
Avoid
Revision Surgery with Satisfactory Results
Regarding
eyelid surgery, there is a widespread perception that "inline or in-outline is natural, and semi-outline or outline is fanciful. However,
semi-outline or outline is only the answer for some patients, and depending on
the individual eye structure, in-outline may look fancier and more natural. In
contrast, the photos above show that a semi-outline or outline may look more
awkward.
It is
essential to diagnose the patient's skin thickness, oculi muscle force,
Mongolian fold condition, and facial structure in detail to make a surgical
plan to meet the patient's needs for 'big and refreshing eyes' and only an
accurate surgical procedure according to the diagnosis will increase the
patient's satisfaction. A skilled surgeon with years of surgical experience and
expertise can perform this series of operations. This detailed process will
prevent the patient from undergoing revision surgery due to dissatisfaction.
