Symptoms and Causes of Untied Double Eyelids (Revision Case)

During consultations, many patients want double eyelid surgery revision for various reasons.

There are many causes, such as multiple creases, untied double eyelids, bent double eyelids, too high or too low eyelids, or dissatisfaction when the result differs from the desired eyes.

Today, we will learn about the cause of ‘Untied Double Eyelids,’ which accounts for many revision surgery cases, and the before and after actual surgery cases.

Symptoms When the Double Eyelids are Untied

1.     1. If the line is ultimately released, one or both eyes will be free of eyelids, returning to the preoperative state.

2.      In case of partial release, the lines will look disconnected and short. The eyelid may also appear to bulge out partially.

3.     In the case of the existing double eyelid, with a higher line, the asymmetry will look worse, and the eyes will look sleepier.

4.     Multiple eyelid lines will be created, causing a messy and untidy look.

5.     The lines are unwrapped and reappear occasionally. Multiple creases might appear, or the line might be pulled up.

If these symptoms appear, you can suspect that your double eyelids have untied.

Then, what is causing it to be untied?

There are mainly four reasons,

1) Thick Skin or much soft tissue 2) Weak Oculi Muscle force 3) Protrusion of the eyeball 4) non-incisional surgery with disconnected or untied adhesion threads.

#1. People with thick skin or a high rate of soft tissue

Thick eyelid skin increases the distance between the eyelid skin and the eyelid plate.

This increases the range of fixation during eyelid surgery, making it more difficult to achieve proper fixation and increasing the chance of the line unfastening.

è  Remove sagging skin and soft tissue through incisional double eyelid surgery.

In these cases, the revision surgery can be performed with a small incision to remove excess muscle fat and other obstructions.

If there is a lot of soft tissue or sagging skin, an incision can be made to reduce the risk of the eyelid untied.

#2. Weak Orbicularis Oculi Muscle Force

In the case of having a weak oculi force,

The line may not be clear and look untied because the eyelids cannot pull upward, in which case an additional correction is needed to adjust the tension of the oculi muscle.

#3. Severe Protrusion of the Eye

In the case of protruding eyes, there is a high risk of the eyelids being easily undone, or sausage eyelids, due to the tension of the skin covering the protruding eyeball.

When performing the surgery, the line should be placed in an in-out shape with a slightly lower-than-average thickness to alleviate the protrusion of the eye, as putting the line too high can make the eye appear more protruding,

Secure the anchoring points at a moderate line, but use multiple points to prevent the eyelid from drooping.

#4. Snapped or untied threads

It is the case where the crease is gone using the non-incisional method, in which the thread that bonds the eyelid skin to the tarsal plate unties or snaps before the adhesion occurs. Therefore, securing the line with the correct solid strength is essential.

In recent years, to create the perfect double eyelid line that doesn't untie, we find the correct supporting points, secure them with a double knot, and remove enough "distractions" such as unnecessary soft tissue and blepharoptosis to create a clear and beautiful double eyelid line.

Sometimes, the line looks untied because it is buried under the eyelid,

making it look untied even though there isn't any problem.

The eyebrows, eyelids, etc., have sagged and covered the eyelid line.

Most people think the line is untied or lowered and wish to get a revision eyelid surgery, but in this case, simply lifting the sagging tissue with the forehead lifting procedure will raise the eyelid line and give you a more precise look.

Today, we briefly looked at the symptoms and causes of double eyelids and how to improve them.

When trying to solve the problem of double eyelid loosening through revision surgery, it is essential to accurately identify not only the initial surgery method but also the factors that could be the cause, such as the current eye condition, so that the same problem does not happen again through appropriate solutions.

Revision may be possible using non-incisional or natural adhesion, but in some cases, an incision may be required, or additional surgery, such as ptosis correction, may be required.
