The Causes of Aged-Looking Double Eyelids (Multiple Creases) and How to Correct Them

The level of wrinkles, elasticity, and hollowness determines your age, especially your eyes, which can often give away your age no matter how much you care for them. That is why hollows above your eyes, irregular eyelid lines, or multiple eyelid lines can make you look older as you age.

Multiple creases, also known as " pupa wrinkles," often appear after age 30; when we begin to age, many people think of it as a complex because it can make you look older and look drowsy and exhausted.

The Cause of Multiple Creases and Ways to Improve it?

Multiple creases have many causes, but the most common is sagging eyelids due to skin aging. In contrast, others include weak eyelid muscles, excessively thin eyelids or little eyelid fat, and excessive removal of muscle or fat under the skin after double eyelid surgery.

Double eyelids are formed by adhesions between the muscles that open the eyes and the skin of the eyelids. Still, suppose the skin is very thin, or due to an acquired condition, there is no adequate muscle or fatty tissue between the muscles and the skin, resulting in multiple adhesions.

In general, double eyelids can be improved by creating a more secure eyelid line, increasing the strength of the eye-opening muscles with ptosis correction, removing sagging skin tissue, and in severe cases, fat grafting to the upper eyelids. Depending on the various causes of double eyelids, the right solution can naturally improve the aged appearance of multiple creases.

Multiple Crease Improvement Case

1.    Weak Orbital Muscle

BEFORE

AFTER

This patient, who underwent blepharoplasty at TS to correct her aged-looking multiple creases, had a weak orbital muscle force with blepharoplasty. At the time of surgery, she had some skin aging and had lost elasticity in her eyelids and under her eyes.

This patient had previously had double eyelid surgery; however, over time, the orbital muscle has weakened, resulting in multiple creases and making her look older due to the bulging fat under her eyes, which is why she wanted to improve her appearance at the same time.

Therefore, incisional ptosis correction was performed to increase the tension of the orbital muscle and, at the same time, incisional ptosis correction, epicanthoplasty, and under eyelid fat reposition to improve the frontal crow's feet, which causes a dull and aged image.

BEFORE/AFTER

You can see the natural improvement in the dull, aged under-eye area by building up the orbital muscles to improve the multiple layers of eyelids into a single clear line and by performing a combination of under-eye fat repositioning, which involves pulling and securing the orbital septum and internal fat to flatten the bulging under-eye fat.

2. Excessively Thin Eyelids or Low Eyelid Fat

BEFORE

PRE-OPERATIVE STATUS

This patient had the typical multiple creases that were caused by a combination of the standard aging process due to the drooping of the upper eyelid and loss of skin elasticity, so she wanted to improve the appearance of the crease and the fat under the eyes that made her look older by having the double eyelid surgery.

In this case, incisional double eyelid surgery can create a natural, defined eyelid line. In contrast, upper eyelid fat grafting can provide adequate volume to the eye sockets to correct existing multilayered adhesions and prevent future reattachment of the double eyelid.

BEFORE/AFTER

At the same time, under-eye bags and puffiness can be treated with under-eye fat repositioning to improve under-eye elasticity and naturally improve the aged appearance.

Find the Exact Cause to Improve the Multiple Creases

Therefore, if you are currently concerned about your multiple creases and want to improve the appearance of your eyes that make you look old and tired, it is recommended that you first consult with a plastic surgeon who is experienced in performing eye surgery to get an accurate diagnosis of your current eye condition and the cause of your multiple creases, and then get a suitable solution.
