Cases of Sagging Breasts Corrected by Only Inserting Implants

Hello, I am Dr. Seo Jeong-Hwa of TS Plastic Surgery.

Most patients who visit us for a breast surgery consultation tend to have rather slim figures and want to enlarge the volume of their small breasts. Among the many, some want to improve the sagging of their breasts for various reasons. While aging can cause sagging due to loss of elasticity, it can also be caused by unhealthy daily habits, and most of all, there are many cases of sagging breasts that shrink after pregnancy, birth, and breastfeeding.

Causes of Sagging Breasts

1. Reduction of Elasticity due to Aging

2. Pregnancy, Childbirth, Breastfeeding

3. Abrupt Weight Changes

4. Daily Lifestyle Habits

Hormonal changes and weight gain during pregnancy, childbirth, and breastfeeding can cause glandular tissue and fat to increase. As you stop breastfeeding and gradually lose weight, you may experience decreased breast size and sagging.

If the skin and soft tissues have adequate elasticity and the change in size is not too severe, the breasts may return to their previous state, but in many cases, the stretched skin will not shrink back. Not necessarily due to pregnancy or childbirth, many women also experience sagging breasts after losing much weight through dieting.

The breast tissue sits on top of the pectoralis major muscle and has several ligaments that hold it in place. When the breast sags, these structures stretch, making it difficult to reshape the breast without surgical correction.

Classification of Sagging Breasts

Breast sagging comes in different degrees and shapes, and there are different surgeries available to improve it. Breast sagging is categorized into three grades: grade 1 with mild sagging, grade 2 with moderate sagging, and grade 3 with severe sagging, each with different surgical procedures and results.

Sagging breasts that can be Improved with Implants Alone

Depending on the severity and shape of the sagging, a breast lift may be necessary, but in some cases, just the insertion of an implant can provide volume and improve the appearance of sagging.

In cases where there isn't much volume on the breasts, the breasts may appear saggy even if they aren't, and are characterized by a high base line with more stretch at the bottom than the top, centered around the nipple.

In other cases, it is when the degree of sagging is mild, and generally, if the saggy appearance is improved when the arm is raised to the side, the implant alone can improve it to some extent. If there is sagging, but the amount of upper breast stretching relative to the nipple is relatively small, it can often be improved when the nipple is not facing down.

Sagging breasts are best addressed in an in-person consultation, as the level of improvement in sagging and the patient's tolerance for scarring can determine whether a mastopexy or implants alone will be sufficient.

In order to improve sagging breasts with implants alone, the surgery should be planned to lift as much of the sagging underneath as possible while leaving as little stretch as possible in the upper part of the breast. The more volume and projection of the implant, the better the correction, and the skin will stretch to accommodate the implant.

However, your individual chest size and body shape should be taken into consideration when choosing an implant, so you can't just choose a larger implant, and should be measured and discussed with your surgeon during your consultation.

When you lie on the operating table during surgery, your breasts will look less saggy because they are not affected by gravity. If you lack surgical experience, shaping while lying down may leave your implants on top with your breasts on the bottom and still sagging.

It's best to find an experienced surgeon who can help you determine if implants alone can improve your sagging breasts with accurate measurements before surgery and honest communication about your expectations for improvement and scarring.
